Regenerative Therapy for Women’s Pelvic Health in San Diego

Shockwave and EMTT are known for tendons, heels and shoulders. The same tools, used by a Doctor of Physical Therapy, do something useful for a problem far fewer clinics will touch. We are the only clinic in San Diego County with all three — focused shockwave, radial shockwave and EMTT.

Scar tissue that will not glide. An abdominal wall that has been guarding for a decade. A tailbone that has not forgiven a fall or a delivery. This page is about how we use these machines for women, and where the honest limits are.

The problem these tools are actually good at

Most of what gets called a pelvic problem is treated as a muscle problem. Squeeze harder. Do the exercises. Come back in six weeks.

But for a lot of women the limiting factor is not muscle strength at all. It is tissue quality — circulation, mobility, how well one layer slides over the next. Years of guarding, a surgery or two, and scar that knitted down to what sits around it.

You cannot exercise your way out of tissue that has gone stiff and poorly supplied. That is the gap these machines fill. They change the tissue, so the exercise has something to work with.

What we use, and where

Radial pressure wave

A handheld applicator sending pressure waves across a broader area — the lower abdomen, a C-section line, the muscle wall, the glutes. Our first choice when the target is wide rather than pinpoint. An FDA cleared radial pressure wave/EPAT device for treating acute and chronic musculoskeletal pain.

Focused shockwave

The same principle, aimed deeper and far more precisely at one small target — a single adhered band, or a stubborn tailbone. Used when we know exactly where we are going.

EMTT

Magnetic field therapy. No contact and nothing to feel. Run alongside the other two to help tissue respond, and the easiest of the three to tolerate on a sore region.

We will always tell you which one we are using, where, and why. If none of the three is the right call for you, we will say that too.

What we see women for

Pain left behind by endometriosis

We do not treat the disease — your physician does. We work on the adhesions, the restricted movement and the guarding that surgery does not resolve.

C-section scar and diastasis recti

The visible line is the least interesting part. We work on the layers underneath, and on whether the abdominal wall can hold pressure again.

Bladder leakage and urgency

Tissue quality and timing, not a hundred more Kegels. Often the fastest-changing complaint on this list.

Tailbone pain that will not settle

Small, deep and stubborn, which is exactly what focused shockwave was built for. From a fall, from a delivery, or from a chair and a decade.

Every one of these has a fuller write-up on our clinic site — see women’s health and pelvic care at Sports Performance Physical Therapy.

How a session actually runs

One Doctor of Physical Therapy, one hour, one-on-one. The machines are part of the hour, never the whole of it — hands-on work and a short, specific home plan do the rest.

Most women describe the shockwave as a strong tapping. We set the intensity with you and we turn it down the moment you say so. EMTT you will not feel at all.

Most women notice something inside the first three or four weeks. How long the whole thing takes depends on how long you have had it, and we will give you a straight answer after the assessment rather than before it.

Where the limits are

  • We do not perform internal exams or internal treatment. Everything here is external. If internal work is what you need, we will point you to someone who does it.
  • These machines do not treat a disease. Endometriosis, fibroids and the rest are managed by your physician. We work on pain, stiffness and movement.
  • If you have symptoms that have not been worked up by a physician, that comes first. We will say so on the call.
  • This is not a device you sit on for twenty minutes. It is physical therapy, and it asks something of you between visits.

Questions women ask us

Is shockwave safe for the lower abdomen?

Used by a trained clinician, at settings chosen for the tissue in front of them, yes. We screen for the situations where it is not appropriate before anything is switched on, and that screen is part of the assessment.

Does it hurt?

Most women describe it as a strong tapping. It is adjustable throughout and we work at an intensity you choose.

Can you treat a scar that is years old?

Yes, and those often respond better than people expect. Old does not mean fixed.

Where do you treat?

Chula Vista and Kearny Mesa. Both locations have private rooms for your comfort. Tell us which suits you better when you call.

Do I need a referral?

No. You can book yourself in.
